Management of Pituitary Apoplexy

Abstract

|
Pituitary apoplexy encompasses a complex series of clinical events occurring as a consequence of the fulminating expansion of a pituitary tumor by infarction, hemorrhage, or hemorrhagic infarction of the tumor and the adjacent pituitary tissue.
The
pathological process may be relatively circumscribed producing minimal clinical symptoms. On the other hand, a massive hemorrhagic infarction of a large pituitary tumor may present as an acute medical catastrophe. The author reports two cases of
pituitary apoplexy he has managed recently and reviews the pertinent literature. Pituitary apoplexy is an eminently reversible cause of acute visual failure and ocular motility defects. Either delay in diagnosis or misdiagnosis remain common in
clinical
practice. Surgical treatment should be considered, especially when vision is severely affected.
